Freem.. at first i seriously thought you were joking since i thought the entire mouse was upsidedown (ball/optical light facing up, buttons facing down) and just could not imagine how that would work.
Now you might as well speak in some other language .
Perhaps a picture, or a better discription would help out.
Btw, if you hold your mouse backwards, doesnt it suck that you have no wheel? At school the mouses have no wheel (iMac.. ikky) and i just can not stand using those comps (and they have no buttons either!!).
And if anybody wanna know, when i hold my mouse (logitek optical mouse). I put my thumb, pinky and ring finger on the side. I never use them. My first figer after the thumb is rested on the wheel when im doing anything that requires scrolling, and right button when i have to right click stuff.. My middle finger is rested on the left click button.
[EDIT]: Im a lefty as well.. So its only natral that i use my left hand for my mouse. Sometimes i use my right hand, its not the same feeling (its weird after using your left hand for the past .. 4+ years).

I use MS Optical Trackball, I like having control with my thumb, but the ball get's sticky if I'm playing games (Sweaty Hands) I use Index for left and extra left (Back Button), Middle for mouse wheel, ring finger for right and extra right (forward button) and my pinky rests on my desk so it doesn't get tired (It is the littlest finger you know, it doesn't need the stress)
As for mouse backwards, I guess you mean cord facing you? how odd, I'ld be clicking everywhere by accident from heavy finger or pushing the mouse.

i had a chem teacher who used to use mouse up side down. Well we made fun of him..... he was wierd anyway (sorry about that)

No, but I upgraded a system for someone once and threw on a copy of Sports car GT and they said the controls are backwards.
She was using the joystick backwards! That is, with the cord running towards her and with the trigger facing her!
That's a dim bulb.

Been using the Logitech TrackMan Marble trackball ever since it came out circa 1992 I think. I have disproportionally short fingers compared to my thumb which makes the trackball fit just right! I got the newer one a few years back when the center button turned into a wheel......hmmmm too uncomfortable...went back to the regular three button.
I love the mouse aiming I can do with my thumb!

Wow, Delta that is a nifty looking mouse. It probably would be great for a laptop.
I tried turning the mouse around, lol, I can't function that way.
