Mouse out of synch

My mouse appears to be 'out of synch' with the monitor!!

If I want to click, say, the minimize icon, the mouse has to be offset by about 1cm from the icon. Similarly if I want to insert a character into text, the I-beam has to be offset by about 1cm from the correct position.

Any ideas as to what is causing this gratefully received!!
