Should pop up in the console
If you dont have the commands right its

    In Console -
    Timedemo 1 <enter>
    Demomap Demo1.dm2 <enter>

I usually disable sound when I framerate test. console commands:

    s_initsound 0
    snd_restart
    timedemo 1
    demomap demo1.dm2

When the demo is done, the console should pop up all on its own, and the bottom few lines should contain the framerate.

I wouldn't disable the sound. That gives you a false reading. Unless you're trying to test if your sound card is a resources drawback. If your FPS are drastically different with and without sound, then you may want to invest in a new sound card. If they're fairly close (within 10% is my personal tolerance), then you're probably ok.

Of course, I'm always testing for raw video performance, not the system as a whole. If you are comparing video cards, disable sound. If you are comparing entire systems, leave it on. Just realize that the sound affects the number you are getting.
