Often when running 3D apps, my comp will crash to the desktop with the standard "illegal-operation" message for the process "ddhelp" in "nvdd.dll" (I think). It happens randomly, but especially when moving from 3D to 2D. Neither the card or the CPU are overclocked. My OS is Win98 and I'm pretty sure my drivers are in the 21.xx series (Det 4). The rest of my specs are in my sig. Can anyone help me?

Possibly a Detonator DLL has been corrupted/overwritten
What version of DX have you got installed?
If it's not the latest then try that
Also reinstall your Nvidia drivers afterwards

I should have mentioned that I've reinstalled many versions of drivers, and my system has been reformatted since this started (I think. I don't exactly remember when it started). I have DX 8.1 release (not beta). I can tell this has something to do with Direct Draw (since the DD test crashes if I try to run it after this error.) Could it be an incompatibility between my mobo drivers and DX or DX and my video drivers or my video drivers and my mobo drivers? (I bet you're confused now. ;))
What make/model are your GeForce and mobo ?
Try to disable Fast Writes in BIOS, and set AGP to 2x, if possible.
If you're saying OS re-install didn't help, I'd check the videocard itself. Try it on other computer, if possible, also try another GeForce in yours.
Best Regards ...
