Network Site Survey Program

What are most people using for network site survey template forms/programs ? Could anyone recomend any applications? To be more specific . I am looking for a program where I could input workstation/server /gateway/routers (etc) locations in companies. Where I could input there IP scemes and other notes on there network.

I suppose this could even be a printable template I could carry with me that I could just fill out manually?

Microsoft Visio 2003 has an IT pro add-on (a program called LANsurveyor) that does it for you and generate the Network and the info into a Visio Network Diagram.
