Help with Boca 33.6 modem

does any one know where i can find the driver for this ISA 33.6 Boca modem?
i tried the driver.com but it doesn't help

I have the same modem in the 486 im having problems with.And I have the origional disks is there any way I can send it to you by email or something?

the program cannot detect my modem for whatever reason and i have no idea what version number of my modem is
