I'm trying to activate the USB port on my AMD K6-3D now / Gainward 5VPA motherboard. All the drivers, etc. installed, but, I couldn't get the system to recognize my Visioner OneTouch 7600 USB scanner. I checked the System area in Control Panel and found an ! next to the USB Root Hub. I checked properties and found this error message:

"Due to hub transfer request failures, a hub has been disabled. Check your computer's cabling and devices, then restart your computer."

I rebooted several times with the USB cable connected and unconnected and after disabling and renabling the USB through the CMOS setup. Issue remained.

are you using win95osr2 or win98?

I am going to bet you have a conflict with your AGP card and it doesn't show up in the device manager.

I also have an issue with USB on my FIC 503+ with an AMD k6-2 CPU - the USB works great but when it is enabled in BIOS my AGP does not do direct 3D - it was working for a short while, but quit for some strange reason. I have ALL the latest patches and drivers, etc. but it remains an unresolved issue. If I disable USB in BIOS my AGP works perfectly. I'm betting this has something to do with why my TNT card bit the dust.

These types of issues are what drove me to the point where I recommend against non-intel chipsets...sadly enough.

For those with a similar problem I finally found out the problem - no free IRQ's. I wound up disabling on board COM ports and removing my modem (I'm hooked up via cable). If only the error messages were a bit more descriptive!
