Hi everybody. As you all can see, I'm from Brazil, therefore english is not my native language (no, it's not spanish either :p ). I'm trying to write this computer program for a friend who lives in the US. He owns a store where he fixes tvs, vcrs, dvd players, microwaves, etc, etc. For the first part of my program, where he has to fill in the fields with the customer's information, I used the sentence "Client's information". Now I'm at the part where he has to fill in the fields with whatever a customer brings in (could be a tv, vcr, microwave, etc). My question is: what sentence could I use for this part? Something like ______'s information. Thanks a lot and sorry for my lack of vocabulary :D

You could use 'Item for repair' or 'Component for repair'
leave a space to write in, DVD or VCR or TV, whatever it happens to be, and then,
Serial Number............ leave a space for the serial number to identify the exact component.

Just put "Job Description" as it might not be a repair, it could be and upgrade or adjustment.
