I've swapped the motherboard twice, installed 4 different floppy drives, checked the bios, checked the cabeling and I still get Floppy disk(s) fail 40
The motherboard is a BCM vp1541

It looks like a defective floppy controller but... I would also consider if there is something underneath the motherboard (right under the floppy controller) that is making short. Look at the floppy controller and see if the connectors are bend. Good luck!

does the light on the floppy stay on all the time?If it does you need to reverse the cables.Have you changed to a different cable?I assume this is what you mean when you say "checked cabling".
