It works fine for over one year, and i dont recall doing anything which might cause this problem, i have check the controller for floppy disk and its shown as working proberly. but everytime i start computer it said "Floppy Disk Failed"
Can anyone give me some suggestion ? Do i have to buy a new floppy disk?

Probably. Just to be sure, though - check all the cables.
