What do you mean "it came up with frequency out of range"?! What does the computer do? Does it boot? If indeed the card came defective, complain and replace it ASAP

Were you overclocking it prior to it's failure?

Have you tried the card on other computers? If you didn't overclock the card and it simply met it's demise only 2 months after the purchase, my first (and only) advise is to return it and demand a new one. Now, what the heck happened to the monitor?

I cant test the card any where else. this is the only puter i have with an agp slot.
im just going to replace the monitor so im not to worried about that.

OK, so my advise is definitely take the card to the shop and let them confirm it's dead...and get a new one :) BTW, go for a 9600 if you can put in some extra 50$ ;)
