I Have installed a new modem into my computer and the computer is not recognizing it and when i go on to control panel, modems, add new modem, and it is not coming up with it. i have also lost the drive cd i think that it is an Intel HaM Data Fax Voice but i am not sure

It's either not fully seated or it's a bad modem. Try switching PCI slots and see if that does the trick. If that doesn't help, exchange it for a new one.

Remove the card first out of the PC. Go into safe mode and REMOVE all of the modems and modem drivers. Shut down the PC and install the card then restart and Windoze will find the card unless its a bad card.

Is device manager showing any 'unknown devices' ?
Which com ports are installed?
Which OS and Mobo?
