Editable PDF

Hi -

I want to make a PDF that a person viewing it can enter in information and then print it. Info like name and phone number. It is not necessary to be able to save or send this info to a db.

Is this built into Acrobat? Is a separate app needed?

I know that it's not what you asked for--since you specified PDF format--but you can create a template in Word that provides roughly the same function. Lock down the text and form detail, even password protect it for editing, and only allow access to the fields you specify. Best part is that it's free...if you have Word that is.
